Q3 2026 saw 𝗠𝗲𝗱𝗶𝗰𝗮𝗶𝗱 𝗽𝗮𝘆𝗺𝗲𝗻𝘁 𝗿𝘂𝗹𝗲𝘀 𝗿𝗲𝘄𝗿𝗶𝘁𝘁𝗲𝗻 and 𝗯𝗲𝗵𝗮𝘃𝗶𝗼𝗿𝗮𝗹 𝗵𝗲𝗮𝗹𝘁𝗵 𝗮𝘀𝘀𝗲𝘁𝘀 𝗿𝗲𝗽𝗿𝗶𝗰𝗲𝗱 as a result. Centers for Medicare & Medicaid Services (CMS) proposed limits on the provider taxes states use to fund their share Medicaid (estimated to reduce federal Medicaid spending by $𝟮𝟰𝟲 𝗯𝗶𝗹𝗹𝗶𝗼𝗻 over 2026–2035), while eligibility continued to contract ahead of the 𝗝𝗮𝗻𝘂𝗮𝗿𝘆 𝟭, 𝟮𝟬𝟮𝟳 work-requirement deadline. State-directed payment caps and 340B payment cuts added further pressure.
Distress defined several of the quarter’s larger provider transactions. Spero Health acquired CleanSlate Centers, keeping a provider with more than 65 centers from shutting down. BayMark Health Services announced completion of a restructuring that hands control to its lenders. A $610 million unrated municipal bond issue intended to finance Clearview Capital, L.P.’s sale of Advantage Behavioral Health failed to sell even at yields up to 𝟴.𝟮𝟱%.
